Financial Options

1. Payment by Patient at the Time of Service: We accept cash, check*, Visa, MasterCard, American Express & Discover. *There will be a $25.00 charge to your account for returned checks.

2. CareCredit Patient Payment Plan: Financing through CareCredit offers a 6 or 12 month same-as-cash plan, or an extended 24, 36 or 48- month payment plan. If you choose the 6 or 12 month plan there will be no interest charged. The extended payment plan is set up on low monthly payments which do accrue interest. Please note that this office is not affiliated in any way with CareCredit. We provide you with information about this company only as a courtesy to you.

Insurance Coverage

We do not accept Insurance as a form of payment. Payment in full is due at the time services are rendered. We also do not file any insurance of any kind.  You will need to file insurance on your own directly with your insurance provider to get reimbursed after you pay for your treatment with us.  While dental insurance is certainly beneficial, as a general rule dental insurance plans usually only pay a small portion of periodontal procedures. We do not file secondary or medical insurance. Our practice will provide you with a treatment plan that explains all costs that may be incurred that will aid you in filing you claim. All treatment plans are valid for 6 months.

Appointments & Cancellations

Each individual appointment requires specialized time and preparation to offer you the best possible care available. This time is reserved especially for you. Therefore, we require all of our patients to give a minimum of 48-hours notice in the event that they are unable to keep their scheduled appointment. Patients who fail to keep their appointment, and do not give at least 48 hour notice of a need to cancel the appointment will be charged $85.00 per hour as per our office policy. Please note that repeatedly canceling appointments or failing to arrive for your appointment at the scheduled time may result in our discharging you from our practice.
